Technical description
The ANTERION from Heidelberg Engineering is a
high-end anterior segment OCT, very different from
a conventional retinal OCT: it is a multimodal SS-OCT
platform dedicated to the cornea, lens, biometry, and IOL calculation.
Heidelberg presents it as a modular, all-in-one solution combining topography,
tomography, biometry, and IOL planning.
Embedded OCT Technology
► Source Type
Swept-Source OCT (SS-OCT)
Wavelength: 1300 nm
This longer wavelength compared to conventional SD-OCT (~840 nm) allows for:
better penetration into opaque tissues, complete visualization of the anterior segment,
less dispersion at the corneal/scleral level, and better penetration of dense cataracts.
► Scan Speed
50,000 A-scans/second
This results in: rapid acquisition, fewer motion artifacts, and improved biometric reproducibility.
2. Anatomical Analysis Field
The Anterion allows for cross-sectional imaging of: tear film/epithelium,
anterior cornea, posterior cornea, total pachymetry, iridocorneal angle,
anterior chamber, iris, anterior and posterior lenses, visual axis,
axial length (with cataract module)
=> In practice, the entire anterior segment is visualized in a single longitudinal acquisition.
3. Corneal Topographic/Tomographic Data
Scheimpflug-like Acquisition but with True OCT
Unlike the Pentacam, which reconstructs using Scheimpflug photography,
the Anterion uses direct OCT measurement.
Number of points analyzed
256 x 65 = 16,640 data points
to reconstruct the anterior and posterior corneal surfaces
Usable topographic diameter
8 mm diameter of topographic/tomographic data is sufficient for keratoconus, post-LASIK ectasia, toric analysis, asphericity, and baseline corneal HOA.
Maps provided: anterior/posterior keratometry, pachymetry, anterior elevation, posterior elevation, total corneal power, epithelial thickness (depending on software/version), irregularity index
4. Integrated optical biometry
With the Cataract App module: simultaneous measurements of: Axial length, K1/K2/Km, ACD, LT (Lens Thickness), CCT, WTW, pupil, visual axis
The key advantage: all these values are visually verified on a real OCT image, not extrapolated solely by interferometry. Heidelberg emphasizes this point: “true measurements, fewer assumptions.”
5. IOL Calculations
Compatible with: Barrett Universal II, Barrett Toric, Barrett True K, Barrett True K Toric, Haigis, SRK/T, Holladay, Hoffer Q, Olsen (depending on license)
Widely used for: standard cataracts, post-refractive surgery, irregular corneas, toric implants.
6. The 4 Software Modules (“Apps”)
ANTERION is modular.
① Imaging App (standard): HD anterior segment scan, angle, lens, anatomical visualization
② Cornea App: complete topography/tomography, pachymetry, elevation, ectasia, keratoconus
③ Metrics App: anterior chamber measurements, angle-to-angle, scleral spur-to-spur, vault, ICL/phake sizing
④ Cataract App: biometry + IOL planning
7. Particularly powerful measurements of the Anterion (what sets it apart)
✔ Direct OCT measurement, not photoreconstruction, therefore excellent accuracy on: operated corneas, opacities, advanced eratoconus.
✔ Very good for post-LASIK/RK/PKP
✔ Excellent for ICL sizing thanks to angle-to-angle measurements and estimated sulcus.
✔ A single device for: topography, tomography, biometry, AS-OCT => clinical workflow savings.
✔ Excellent DICOM/Heyex2 integration.
In summary, the ANTERION is ultra-clinical, offering SS-OCT 1300 nm + 50,000 scans/sec + true corneal OCT + biometry + IOL calculation + anterior chamber measurements on a single platform.
It is currently one of the most comprehensive devices on the market for: refractive surgery, premium cataract surgery, keratoconus, ICL, and complex corneas.
